# Form Tabs
When Adding a Form Tab, you need have your form created and saved, therefore you will click on **#1**** Form Tabs** tab. If you have Tabs existing for the specific form, they will then be displayed in a grid form in the **Form tabs** section. Therefore click on the **#2** **Add new** Button.
The form to add a Form tab will then be displayed, where the field in **#1** is the Form tab **Display Name**, **#2** is the **Unique Name** of the Button you are creating, **#3** is the **sort Index** which allows you to place your Form tabs in a specific order therefore, On the **#4** field is the prepopulated Form name, which you are trying to add the form tab in.
